Check in: 8am-9pm
One representative from each team should come to the check in with the following:
- Completed roster. Every player needs to be a current USAU member and have a signed waiver on file. your roster should match the one I am printing out tonight from USAU.
- Medical Release: The chaperone should have a signed medical release for every player on the roster. The chaperone will keep the medical release form with them at at times during the tournament.
- $5 for each player on the roster.
If you have players that are not current members or do not have a signed waiver then you MUST take care of it before they can play. If you have roster issues please show up early to take care of it. I will not hold up games.Players will need the following:
- A receipt proving they have paid their USAU membership. If you can’t prove it then you will need to sign up and pay your $29 again.
- A signed waiver. If you don’t have it you will need to fill one out and get it signed before you can play.
Please everyone do your part to make this as painless as possible. If you don’t have what you need, GET IT BEFORE YOU ARRIVE. All of the forms are on the USAU site and their are links to all the documents on the tournament site.
